Receive Community Service Hours

1) Fill out all the information on your Community Service sheet that you can before submitting this form. The only portions that should not be filled out are the signatures needed from Free Peer Tutoring. If you do not remember the dates or lengths of your past tutoring sessions, please refer the confirmation emails from submitting the "Turn in Tutoring Session Recordings" Form.

2) Attach this partly filled out sheet to this form as a PDF. Other types of files will not be accepted.

3) Briefly describe what we need to do to confirm your Service hours. If your school has any special requirements, let us know. (Ex. "for my school, you must sign both the front and the back of the sheet").

4) We will digitally sign your form and email it to you once we confirm your hours. We have a lot of students wanting to receive Community Service hours, so please be patient.
